In order for us to provide you with an accurate quote for our services, we recommend filling out all sections of this form to the best of your ability. Upon receipt, we will review it and call or email you back with a quote for services. This quote may be reflected as a price range rather than a fixed price; once we provide your 1^st service, the price will become fixed moving forward if you are requesting additional services. Please be aware that your first service must be paid C.O.D. and we can move forward with a written contract upon your request. Please note that our trucks are the size of trash trucks and we perform the service with hoses running to the grease trap.
